Sign up at the library EVERY TUESDAY evening from 6-7PM. PLEASE BRING PROOF OF INCOME so that we can determine the level of assistance that you qualify for such as check stub, last years taxes, bank statement, etc. If you are on public assistance a MEDICAID CARD or FOOD STAMP CARD (EBT) is acceptable and automatically qualifies you for maximum level of assistance. Also be PREPARED TO PAY FULL AMOUNT DUE at sign up because you are issued a voucher to take with you to the vet that serves as your payment for the procedure.

If pet is not current with rabies vaccination there will be an additional charge of $7 for vaccination making your total cost $32 (a savings of about $100).
